December 1, 2022
Dear Patient:
It is with mixed emotions that I am announcing my retirement. I will no longer be seeing patients in the office effective December 31, 2022. After that, I plan to be available for Memorial Women’s Specialists televisit consultations and to assist other doctors in complicated surgeries. It has been a great pleasure providing for your healthcare needs over the many years, and it is not easy for me to give it up.
Dr. Mina K. Sinacori and Karol Rivera, MSN, APRN, FNP-C will be taking over my practice. I am pleased that you have the opportunity to have them available for your OB/GYN healthcare needs. Dr. Sinacori is a well-trained graduate of Baylor College of Medicine. She completed her residency in Houston via the Baylor Affiliated Hospitals and is happy to continue your care with Memorial Women’s Specialists. Karol Rivera is a highly experienced Nurse Practitioner. She will be available for most obstetrics and gynecology visits, and is already seeing many of you. I am glad to leave my patients in their capable hands.
Your medical records are confidential, and will remain on file with Dr. Sinacori and NP Rivera at Memorial Women’s Specialists. Should you choose to transfer to another practice, your records can be released to you or another person you designate, with written permission.
IF YOU HAVE AN APPOINTMENT ALREADY SCHEDULED WITH US FOR NEXT YEAR, YOU WILL BE SEEN BY ONE OF OUR OTHER PROVIDERS UNLESS YOU CALL TO CANCEL IT. Memorial Women’s Specialists looks forward to continuing your care.
I have greatly valued our relationship, and I thank you for your loyalty and friendship over the past many years. Best wishes for your future health.
Sincerely,
The following insurances are in-netowork. If you do not see your insurance listed, please call our office to confirm.
